Sidewalk Estimated Added Quantity = 6 SY @ $66/SY = $396.00 The location for the above items is the intersection of Airport Way/Shattuck Ave S x Add the following lump sum item to the contract: o Lump Sum Item No. 29-1 SHATTUCK REVISIONS Quantity = 1 LS, Lump Sum Amount = $63,202.29 Location is the intersection of Airport Way and Shattuck Ave S Change to Contract Plans: As required in FWD No 17, FWD 20 and FWD No 23, the Contractor is directed to directed make the following plans revisions at the intersection of Airport/Shattuck: 1.) Furnish and install 1 EA Type 8 J-Box (per the WSDOT Standard Plans with non-skid locking lid) at the SW corner of Airport /Shattuck between new ramp and new landing to replace the existing J-box. Furnish and install 1 EA Type 2 J-Box (per the WSDOT Standard Plans) at the NW corner of Airport/Shattuck and 1 EA Type 2 J-box at the new power connection on Shattuck, south of Airport Way. Install 2 EA-3-inch Sch. 80 PVC Conduits (see new Wire Note #17) in the same utility trench as 1 EA- 3- inch Sch. 80 PVC per revised Wire Note #2. See the Plan Sheet TS9.01 Rev1. 2.) Reduce the utility trench construction limits on Shattuck from the existing pole on Tillicum/ Shattuck to the revised power source connection which is the new Type 2 J-Box with non-skid locking lid noted           Change Order No. 29 Contract Title: Lake Washington Loop Trail Contract No. CAG -20-126 Page 3 of 5 above. The Type 2 J-box shall be used to intercept the existing service conduit and conductors and splice the existing power wires to the new 3-#2 wires (Wire Note #2). The Contractor shall coordinate the work with PSE and the City. The Contractor shall provide temporary power to the signal at Airport/Shattuck and street lights as needed for the PSE splice and connection. See Plans Sheets TS9.01 Rev 1, TS 9.02 Rev 1 and C3.11 Rev 2. 3.) The curb ramp and sidewalk on the SW corner of the intersection of Airport/Shattuck shall be removed and waste-hauled/disposed. It shall be replaced with a new ADA-compliant WSDOT Type Parallel A Curb Ramp and sidewalk transitions, per Plan Sheets C2.04 Rev 2 and C3.05 Rev 2 and C311 Rev 2. Contractor shall provide a Ped TCP for this work. 5.) Remove the existing plastic crosswalk lines on the Airport Way Crossing of Shattuck Ave S, on the Shattuck Ave S crossing at Airport Way and at the Airport Way crossing at Logan Avenue N, by sand blasting, per Special Provision 8-22.3(6). The Contractor is directed to replace the markings with thermoplastic markings per COR Standard Plan 127 and Special Provision 8-22.2. 6.) Rewire the Shattuck and Airport Way Traffic Signal poles with new conductor and to terminate conductors in the new controller cabinet. The Contractor is directed to furnish and install two each terminal cans on Poles #3 and #4. The terminal cans shall be natural finished aluminum and shall meet City of Renton Standard Plan 140. The work required for the service cabinet installation, rewiring and PSE service transfer may be completed with multiple stops and starts as required to minimize working days. 7.) Furnish and install two “No Pedestrian Crossing” signs R9-3 (18”x18”) and one “Use Crosswalk” directional sign R9-3bP (18”x12”) on the easterly approach of the intersection, as shown on plans sheet S7-04 Rev 3. Reason for Change: Contract Plans: The original plans showed the point of connection for the signal and illumination improvements at the northwest corner of the Shattuck/Tillicum intersection. Trenching at this location would trigger the need for a utility pole relocation and curb ramp improvements. It was determined that a point of connection was possible to the north of the original location. The T-cut trench restoration work across Airport Way at Shattuck will require a substantial patch within the marked crosswalk on Airport Way. This work triggers the need for a curb ramp retrofit at the SW corner of Airport Way and Shattuck Ave S. This demolition work and reconstruction of the curb ramp at the SW corner of Airport/Shattuck is added to the Contract.           Change Order No. 29 Contract Title: Lake Washington Loop Trail Contract No. CAG -20-126 Page 4 of 5 The utility trench transverse patch crossings at the intersection of Airport Way/Shattuck and Airport Way/Logan encroached on portions of the thermoplastic crosswalk pavement markings at these marked crosswalks, so the crosswalk markings needed to be replaced as a result. The plans called for the existing signal wire to extend and connect to the new signal cabinet at the intersection of Shattuck/Airport Way. The entire signal is required to be rewired as a result. The installation of two terminal cans on poles 3 and 4 simplifies the rewire work. The addition of the no pedestrian crossing signs and use crosswalk directional sign clarifies that there is no pedestrian crossing allowed on the east approach to the intersection. Materials: The terminal cans shall be natural finished aluminum and shall meet City of Renton Standard Plan 140. Plans: Plans Sheets C2.04 Rev 2, C3.05 Rev 2, C3.11 Rev 2, TS9.01 dated 9/7/21, TS9.02 dated 9/7/21 and TS9.03, dated 9/7/21, S7.04 Rev 3, S7.05 Rev 3, WSDOT Standard Plan J-40.10-04, WSDOT Standard Plan J-40.30-04, COR Standard Plan 127. Equitable Adjustment: This Change Order does include an equitable adjustment per Section 1-09.4 of the 2020 WSDOT/APWA Standard Specifications. See attached. Extension of Time: The Contractor agrees that the time for Completion is extended 0 Working Days for the additional work. A time extension for this work has been addressed in Change Order No 11. Sections 1-04.4 and 1-04.5 of the 2020 WSDOT/APWA Standard Specifications shall govern this Change Order. The Work of the referenced Contract is modified to include the changes detailed herein. The payment provided for herein shall constitute the complete and final settlement for all costs of labor, equipment, materials, overhead, profit, permit fees, and all other claims that may be made by the Contractor as a result of this change.
