THE CONTRACT IS MODIFIED AS FOLLOWS:
Description of Change:
Change to Contract Plans:
As directed by email on February 23, 2022, the Contractor furnished and installed wire for the
PTZ camera to be mounted on Pole No 2. As later required in FWD No 32, the Contractor is
directed to relocate the PTZ camera mount and camera from Pole 2 to Pole 3 per the original
contract drawings. The mount to be located oriented toward the east (toward the Sub Shop).
The Contractor shall repair hole(s) in Pole 2 per WSDOT Plan IS-17, as required. The Contractor
has already furnished and installed the required wiring for the PTZ camera at Pole 3 along with
an additional 10 extra feet. The Contractor shall preserve the maximum amount of additional
wire at Pole 3.
Reason for Change:
Contract Plans:
The reason for the change is that the PTZ camera is not functioning properly at the Pole 2
location, due to a voltage drop.

Change Order Cost:
This Change Order does include an estimated equitable adjustment per Section 1-09.4
of the 2020 WSDOT/APWA Standard Specifications.
The Work will be measured and paid under the lump sum price of $4,205.60

The Time for Completion is extended 0 Working Days.

Sections 1-04.4 and 1-04.5 of the 2020 WSDOT/APWA Standard Specifications shall govern
this Change Order. The Work of the referenced Contract is modified to include the changes
detailed herein. The payment provided for herein shall constitute the complete and final
settlement for all costs of labor, equipment, materials, overhead, profit, permit fees, and all
other claims that may be made by the Contractor as a result of this change.
