10. DESCRIPTION/JUSTIFICATION OF AMENDMENT:
Due to less than anticipated staff participation in April trainings,the grant agreement is being extended a month
to provide staff members with another opportunity for emergency management training. There is no impact to
the budget or work plan.
11. AMENDMENT TERMS AND CONDITIONS:
1. Change the Grant Agreement expiration from August 31, 2017 to September 30, 2017.
2. The overall Grant Agreement amount $56,459 remains unchanged.
3. Replace Original Timeline Exhibit D with Revised Budget Sheet Exhibit D, attached to this amendment.
4. Update Article 1, Key Personnel Exhibit A as follows:
5. Update Department Key Personnel from Tirzah Kincheloe, to Gary Stumph, Program Coordinator,
qarv.stumqhCc�mil.wa.aov, 253-512-7483.
This Amendment is incorporated in and made a part of the Grant Agreement. Except as amended herein, all other terms
and conditions of the Grant Agreement remain in full force and effect. Any reference in the original Grant Agreement or an
Amendment to the "Grant Agreement" shall mean "Grant Agreement as amended". The Department and Subrecipient
acknowledge and accept the terms of this Amendment as identified above, effective on the final date of execution below.
By signing this endment,the signatories warrant they have the authority to execute this Amendment.
