This Addendum No. 4, containing the following revisions, additions, deletions, and/or clarifications, is
hereby made a part of the Plans and Contract Provisions (Contract Documents) for the above-named
project. Bidders shall take this Addendum into consideration when preparing and submitting their bids.
Bidders shall acknowledge receipt and acceptance of this Addendum No. 4 in the space provided on the
Proposal Signature Page or by signing in the space provided below and submitting the signed addendum
with the bid package. Failure to provide this written acknowledge may result in disqualification of the Bidder’s
submittal.

SPECIAL PROVISIONS
7. Revise the Special Provision Section 1-03.3 Execution of Contract (Page SP-16) as follows:
Revise the third paragraph as follows in bold:
Within 10 calendar days after the award date, the successful bidder shall return the signed
Contracting Agency-prepared contract, an insurance certification as required by Section 1-07.18,
a satisfactory bond as required by law and Section 1-03.4, the Transfer of Coverage form for the
Construction Stormwater General Permit with sections I, III, and VIII completed when provided.
8. Revise the Special Provision Section 7-10.4 Measurement (Page SP-191) as follows:
Revise the first paragraph as follows:
“Bioretention Planter” shall be measured per lump sum.
9. Revise the Special Provision Section 7-10.5 Payment (Page SP-191,192) as follows:
Revise the first paragraph as follows:
“Bioretention Planter”, per lump sum.
Delete the 6th paragraph as follows:
Top Soil A for bioretention planter shall be measured and paid under the “Top Soil A” bid item.
10. Revise the Special Provision Section 7-21.5 Payment (Page SP-225) as follows:
Revise the bid items in the first paragraph to include:
“Furnish and Install 8 In. Diam. CIPP for Sanitary Sewer”, per linear foot.
Revise the second paragraph as follows:
Payment for furnishing and installing 8-inch or 12-inch CIPP for Sanitary Sewer will be made at
the amount bid per linear foot, which payment will be complete compensation for all labor,
materials, equipment required to complete the work specified in the contract documents and
plans, and shall include but not be limited to the following:
Revise the fifth bullet point as follows:
Furnishing and installing 8” or 12” diameter CIPP sewer pipe.
11. Revise the Special Provision Section 8-35.4 Measurement (Page SP-313) as follows:
Add the following after the fifth paragraph:
“Install __” Diameter Fiber Optic Cable – [Utility Owner]” shall be measured and paid per linear
foot of cable installed when the cable is furnished by Others. The Contractor shall keep record for
which utility the installed cable will be used by.
12. Revise the Special Provision Section 8-35.5 Payment (Page SP-313) as follows:
Add the following after “Install __” Diameter Conduit – [Utility Owner]” per linear foot:
“Install __” Diameter Fiber Optic Cable – [Utility Owner]” per linear foot.

13. Revise the Special Provision Section 8-14.3 Construction Requirements (Page SP-250) as
follows:
Remove Decorative Concrete Finish section paragraph 3 and replace with the following:
Concrete acid stain shall be semi-transparent water-based stains penetrate into the concrete to
attain a faux, layered appearance. The acid stain shall be DIY-friendly, ZERO VOC’s (Volatile
Organic Compounds).
Concrete stain shall be a penetrating, semi-transparent, water-based stain specifically formulated
to color concrete, or other cement-based materials, by delivering ultra-fine pigment to the concrete
surface, for exterior use. Color of stained concrete surface shall reveal a bluish-green color tone.
Concrete stains shall have the following characteristics:
• Low VOC water-based formulation
• Stain application: pump sprayer, mop, stain/paint roller
• Apply top coat sealer with superior adhesion to stained concrete surfaces, 25% solids,
durable satin acrylic finish that is non-yellowing and UV resistant.
Physical and Chemical Properties shall include the following:
•Flash Point > 93.3ºC (200ºF) - TAG Closed Cup
•Physical State Liquid
•Solubility < 70%
•Volatiles < 70%
•VOC 0%
• LVP-VOC 0%
• Specific Gravity / Density ~ 1.11
• pH (± 0.3) 7.5 – 8.5
• Boiling Point >100°C / 212°F
•Molecular Weight (g/mol) Varies
ALL OTHER REQUIREMENTS OF THE CONTRACT DOCUMENTS REMAIN IN EFFECT.
